Food Service Forms

The 2023-24 Free and Reduced Price School Meal information and application may be accessed by clicking on the links below.  When filling out the Free/Reduced Meal Application, it is very important that you make sure to do the following:
 
Step 1
  • List all children in household, grade, and check learner status box if it applies.
  • Check Foster/Homeless/migrant if they apply.
Step 2
  • If you receive benefits from the state such as SNAP, TANF, make sure to put your case number in this section.
Step 3
  • List ALL adult household members, and any income.  Check frequency of income.
  • Also, put number of TOTAL household members in box under the list of names.  All adults and children must be included.
  • Put last four numbers of your social security number, or check no SSN.
Step 4
  • Put address, phone, printed name, signature and date.
 

Without ALL of this information, your application cannot be processed, and may delay your free/reduced status.
